Frequently asked questions
What is an 835 file?
An 835, or electronic remittance advice (ERA), is the file an insurer sends with a payment. It lists every claim the payment covers, what was paid, what was adjusted and why, as codes. This tool turns those codes into plain English.
Are my files uploaded anywhere?
No. 835 files contain patient information, so they are read entirely by code running in your browser. Nothing is sent to insytesBI or anyone else, and closing the page discards them.
Can I upload more than one file?
Yes. Choose up to 50 files at once, such as a month of remittances. They are combined, and a payment that appears in two files is counted once. With more than one month, the tool also spots payers that are denying more or paying slower.
How does it decide a claim was underpaid?
It compares what the payer paid per unit for a procedure code with what the same payer paid for the same code on your other claims in the files, and flags lines more than 15% below the usual amount when there are at least three others to compare with. It is a prompt to check your contract, not a fee schedule.
What does “Check this” mean?
The payer paid nothing on the claim but still listed a copay or coinsurance. That combination is unusual and is often a processing error on the payer’s side, so it is worth a call rather than billing the patient.
Which file extensions work?
.835, .txt, .edi, .x12, .era, .dat and similar. The extension does not matter: the tool reads the contents and tells you if a file is not an 835.
